Implants

Implants are one way of replacing missing teeth. A post is planted in the jaw bone to support a replacement tooth. This acts like the root of a natural tooth. Implants can also be used to support fixed bridges or dentures.  Implant treatment normally has two stages. First, the implant is placed in the jaw. Then, when the jaw has healed, replacement teeth are attached to the implant. In some situations it is possible for temporary teeth to be attached to an implant at the time of fitting.

Single Implants

Case 1 - Delayed Implant Placement

Before

The patient had fractured their upper right Central Incisor so only a small portion of the root was present, they were also unhappy about the discolouration in the upper left Central Incisor. The root was removed and the area was left to heal for 2 months as their was infection present around this area. The implant was placed and left to heal for a period of 4 months.

After
4 months later the implant was exposed, a temporary crown was placed and the soft tissues were left to heal. Then the upper left Central incisor was prepared for a Veneer, impressions were taken for the laboratory. Finally a Zirconia abutment was torqued into the implant which was then fitted with an all porcelain crown, the upper left central incisor had a porcelain Veneer cemented at the same appointment.

Case 2 - Immediate Implant Replacement

The patient had lost their post and core crown from their upper right central incisor, also their upper left central incisor was stained and unaesthetic due to an old composite filling. The root was removed and an implant placed on the same day. The implant was left to heal for 6 months

After 6 months the implant was exposed and after soft tissue healing it was restored with a titanium abutment and a porcelain fused to metal crown, while the left central incisor was restored with a porcelain veneer.

 

Case 3

Patient was missing their upper right lateral tooth, therefore an implant was placed and left to heal for 4 months,  it was then surgically exposed and a aesthetic zirconia abutment was placed on the implant, along with a temporary crown in order to shape the soft tissues.

Once the soft tissues had healed to the desired shape a final impression was taken and a all ceramic crown was fitted.

Two Implants

Case 1

The patient was missing their 2 lower teeth due to a sporting accident. 2 implants were placed and left to heal for 3 months.

After 3 months the implants were exposed and restored with 2 titanium abutments and 2 separate porcelain fused to metal crowns.

Case 2

Patient was missing both of their upper lateral incisors. 2 implants were placed at the same appointment and left to heal for 4 months.

After 4 months the implants were exposed and both were restored with a zirconia abutment and a all porcelain crown.

Implant Bridge

Case 1

Before
The patient was missing all their upper Canines, Premolars and Molars and had been wearing an acrylic denture. We placed 6 implants and left them to heal for 6 months.

After 6 months the implants were exposed and 2, four unit bridges were constructed in the lab and placed on the 6 implants either side of the upper jaw.

Case 2

The patient had a broken down dentition with their remaining teeth requiring extraction. Instead of going for  upper and lower full dentures, we were able to place 4 implants in their lower jaw and 6 implants in their upper jaw.

After 6 months all the implants were exposed, the patient returned for a number of appointments until finally upper and lower hybrid acrylic to metal bridges were constructed. These were then fit on the implants in one appointment.

Full Arch Bridge

Case 1

The patient had a old bridge in place that was decayed and loose. This bridge was removed along with any teeth associated with it. The patient was given a temporary denture to wear. After 2 months 6 implants were placed and left to heal for 6 months.

After 6 months the implants were exposed, a number of appointments followed in order to construct an 8 unit bridge. As the patient had lost so much bone and soft tissue originally pink porcelain was added to the bridge in order to get the teeth at the correct length, producing a harmonious smile.

Case 2

The patient had been missing all his upper teeth and was wearing a full upper denture. 10 implants were placed and left to heal for 5 months.

After 5 months the 10 implants were exposed and after soft tissue healing a number of try in appointments were made to get the teeth in the right position which resulted in a 12 unit bridge being placed

Implant Overdentures

Case 1 - Upper bar

The patient was missing all of their upper teeth and the majority of their lower teeth but their main complaint was not being able to wear a full upper denture due to a gag reflex. 4 implants were placed and left to heal for 4 months.

After 4 months the implants were exposed, a bar and removable overdenture with clips were constructed over a number of appointments. Due to the retention of the bar(in before picture) there was no need for a palate on this overdenture which was very comfortable for the patient and avoided problems with their gag reflex. A lower partial denture was also constructed for this patient.

Case 2 - Lower bar

Before
The patient had been wearing upper and lower dentures for many years, they were unhappy with the aesthetics of the upper denture and found it difficult to eat with the lower denture. 3 implants were placed and left to heal for 4 months

After 4 months the 3 implants were exposed, following a number of appointments a titanium bar (1st pic above to the left) and removable overdenture with clips (2nd picture above) were constructed. A new upper denture was also constructed. This improved the patients aesthetics and they found eating a much more pleasant experience with the lower overdenture.
